Posted on another website. This is putting a lot of the other pieces we have heard together and makes a lot of sense to me.
From someone who claims to know this info secondhand via someone involved with the team...
Hurd fumbles against UGA in the goal line and gets lit up by Butch. Hurd tells him to F-Off and that Hurd is the team and that TN cannot win without him. He was benched. Kamara needed a break so Hurd came back in, caught the pass, and then refused to go back in the game. This would explain why he was still standing on the sidelines with his helmet. If he would have had a concussion he would not have had his helmet (protocol).
The next week he was not at practice M-W (he was not injured as reported). When he came back Butch had the players vote on whether or not he could come back. They agreed to let him back but did not think he should be allowed to play at A&M so he did not make the trip (he was not injured as reported).
At USC Hurd was pissed because they were playing Kelly due to the production he was having and Hurd's lack of production, so Hurd began pouting and refused to go in the game when Kelly needed a breather. Carlin went in and caused the fumble. On the sidelines game after the fumble Dobbs got in Hurd's face and said he was quitting on the team. Hurd refused to go back in the game for the rest of the night (still had his helmet in hand our on while standing on the sidelines). As reported by the program, he was not injured.
Sunday night there was another team meeting for the players to decide what to do about him. Several called him out for being selfish and quitting on the team. Hurd walked out of the meeting and that was that. Hence the story about the transfer request put out by the program.
The team basically challenged him to meet some criteria, prove himself, or get the hell out. So he left.
