OT; Insurance out of Network?

I recently had a procedure. I went to a hospital in network, had a Dr. in network. Then BAM!!! got the Ole' Anesthesiologist Out of Network bill.

I didn't even know I had a choice. Did I have a choice?

Is this typical? I'm pretty peeved about it. Hell, I'm not even sure who to be pissed at?
